Insurance Filing Policies

In order to serve you well and to avoid misunderstandings, we would like to inform you of our office policies regarding the filing of insurance claims. We do not participate with any health share plans and cannot see those patients as “self pay.”

We participate with most major carriers including:

  • Aetna (excluding the Premier Care Network) 
  • BlueCross BlueShield (excluding the E and L Networks)
  • Cigna
  • Humana
  • United Healthcare (excluding the Navigate plan).

This means that we will bill those health plans for which we have an agreement and will only require you to pay your estimated patient responsibility at the time of service. While we are pleased to be able to provide service to you, it is very difficult to keep track of all the individual requirements. We recommend that you contact your insurance company or human resources at your place of employment with specific questions regarding your plan benefits and to confirm provider participation.

If at any time there is a change in your insurance plan please notify the front office staff so that your insurance claims are filed on your behalf in a timely manner.

Private Pay Patients

In the event you do not have insurance coverage and must pay for your services, we require payment at the time services are rendered. We accept cash, checks, Mastercard and Visa and offer a self pay discount. If you are unable to pay in full, please contact our office prior to your visit to make payment arrangements.
